line 3 and 4: Not certain of exact name or time, NBSclan.net server running kf-evilsantaslair at HoE between 11:00pm 2/1/2011 and 1:00am 2/2/2011 (game was sufficiently long that I'm not sure at what time the ban occurred)
line 5: Description of what happened/reason for request - I'm fairly certain the ban occurred because I was playing as a lvl 5 berserker on HoE. I saw the warning about allowed lvls, but I also saw that my team was using the all berserker strategy for the map. I informed the team that I had multiple lvl 6 perks I could join as, including medic for speed, and that my berserker lvl was 5. They asked me to join as a berserker. Naturally, I was both slightly slower than the other berserkers and had less damage resistance, so I died a couple times early on, was doing better late game, and then halfway through the 9th or 10th wave got pinned by an fp attacking one of my teammates. Being pinned as I was, I attempted to attack the fp in the hopes of either finishing it, or distracting it from my teammate so he could get away. I died, and was instantly perm banned. I believe [NBS]_markg was listed as a spectator at the time. I recognize that the rules are in place to insure all players can pull their weight, which is why I wanted to be sure my team would accept a lvl 5. I also recognize that I was not the most useful, but I did the best I could in the situation and tried most of all to be a team player.
